HIGH SCHOOL TRANSITION PARTNERSHIP
Helping Students Build a Direction Before Graduation
Progress Partners Life & Career Development partners with high schools to help students who are uncertain about their next steps develop a clearer direction for life after graduation.
The program combines individualized assessment, student coaching, staff collaboration, progress monitoring, family involvement, and post-graduation planning. The assessment will be turned into an action plan to be used throughout the school year.
The year end goal is for your young adult to have more experience, greater confidence, clearer direction, and a practical plan for what comes next.
September — Discovery & Planning
Review current information, strengths, interests, goals, family input, and school supports. Meet with the student and appropriate staff and develop an individualized Life & Career Direction Plan.
October–December — Building the Foundation
Regular student sessions begin with approximately weekly support focused on goals, confidence, employment readiness, independence, and real-world experiences. A more in-depth progress review is completed in late October.
January — Mid-Year Review
Review progress, identify what is working, make adjustments, and establish priorities for the second half of the school year.
February–April — Action & Experience
Continue regular student sessions and practical action steps. Areas may include employment, volunteering, career exploration, communication, life skills, community involvement, and greater independence. A third in-depth review takes place in late April.
May–June — Transition & Next Steps
Focus on what comes next after the school year. Families receive a year-end summary outlining progress, strengths, areas for continued development, and recommended next steps.
Throughout the year, brief family and school check-ins can be used to keep everyone informed and make adjustments when needed.
Program investment
The program includes the core coaching, planning, progress monitoring, and school collaboration needed to keep the student moving forward throughout the year. Additional services outside the regular program structure—such as extensive employer outreach, community networking, volunteer or internship development, travel, job-site support, or intensive opportunity-building—are available at $60 per hour.
